THE STANDARD | June 2019, Revision 5 11 EXPECTATIONS PERFORMANCE MEASURES 4.1 HSES incidents are properly reported and classified.  Incidents are managed in compliance with the Kosmos Global Standard for Incident Reporting and Investigation 4.2 Incidents are investigated to determine root causes and to identify preventive and corrective actions necessary to prevent their recurrence.  Appropriate level of investigation is conducted with management participation in accordance with the Kosmos Global Standard for Incident Reporting and Investigation 4.3 Preventive and corrective actions from incidents are implemented.  A centralized system is used to track all actions from incidents to completion/closeout 4.4 Lessons learned from incident investigations are shared with appropriate audiences to help prevent recurrences.  Incident investigation reports are distributed to internal and external stakeholders in accordance with the Kosmos Global Standard for Incident Reporting and Investigation  Incidents are reviewed during regular operations and management meetings  Lessons learned from incidents are considered during project planning and operational readiness activities INCIDENT REPORTING AND INVESTIGATION INTENT: Effective incident reporting, investigation and follow-up provide the opportunity to improve HSES performance.
